Whitman Hospital's Health Information Management (HIM) department is responsible for keeping complete and accurate information for each patient. Your original health record is property of Whitman Hospital & Medical Center (WHMC) but the information in it belongs to you. To obtain copies of your medical record, please see below or contact the HIM department at 509.397.3435, ext. 306.
To obtain copies of your health information (medical records) complete the Authorization To Disclose Protected Health Information. Failure to provide all requested information may cause a delay in processing your request. Return the signed authorization form to Whitman Hospital & Medical Center by 1of the following methods:

You can bring your form to WHMC at the front desk and ask for assistance from a HIM representative. Representatives are available Monday through Friday, 8 a.m. to 4 p.m.
Once we receive your request we are usually able to process it within 5 days. If you have questions regarding the status of your request or about completing the form, contact the HIM department at 509.397.3435, ext. 306. To protect your privacy, records cannot be faxed or emailed. A fee may be associated with the procurement of records.
Medical Records can be obtained for a family member if you are the legal guardian or have power of attorney for that person. The patient may also request that records be released to a personal representative. Please contact the HIM Department for more information on requesting records for a family member.

For authorization to release radiology images only, fill out the radiology release form and submit to Whitman Hospital's radiology department directly.
Correction or amendment of medical records
If you feel that there is an error in your health information (medical record), please contact the HIM department.
In accordance with Washington state law, we will review your request and respond within 10 days of receiving your request. A copy of your request will be added to your record.
